The Well Planned Action Series

The Well Planned Action Series will support teams to improve the experience for people creating, clarifying, and communicating medical goals for discharge. Medical goals for discharge provide information about the clinical reason for a patient’s admission and describe the clinical status the team and patient are working towards in order to be medically fit for discharge. Medical goals for discharge is not a checklist to be achieved before discharge (criteria led discharge), or a component of advanced care planning (goals of care).

It’d be fantastic if you could please also share this information with any colleagues/friends you think would be interested.

Action Series Highlights:

  • Flexible, low time commitment – 5 Modules spread over 5 months (November 2024 – March 2025); can be done in as little as 5-10 minute segments, maximum 1 hour per month
  • Up to 6 hours of sessional funding at the JCC rate is available for physicians through PQI
  • Participate in multidisciplinary teams of 3-7 people who are involved in creating, communicating, or clarifying medical goals for discharge – Must include at least one physician and one non-physician role, including Nurse Practitioners, Midwives, Nurses, Allied Health professionals, etc.  Example team compositions: A) 3 RNs, 2 CNLs & 2 Physicians;  B) 2 NPs, 1 Physician & 2 LPNs;  C) 1 Physician, 1 CNE & 1 SLP​
  • There is no “team lead” role and no additional responsibilities for the person who pulls a team together. Reach out to PQI@islandhealth.caif you need help identifying a team.
